Charms and the charm grid
The mistake Diablo II habit suggests is keeping charms in the inventory. The items page says charms work only in the 5 by 4 charm window, not in the backpack and not in the stash, and the mercenary ignores them. I am quoting that rather than testing a grid I cannot open here.

Sizes and the rates the wiki prints
Small is 1 by 1, large is 1 by 2, grand is 1 by 3. About 1 in 134 drops is a charm, and the three sizes split evenly. Item level is reduced by 14 on small charms and by 7 on large charms. Grand charms are not reduced. At item level 99 the gap closes and all three are 99. The page says skillers are about 36% of grand charms at item level 50 and up, and about 1 in 20 of those is your own class tree. Both rates are a single official page. I am not rounding them into a “best charm” list, because the affix table is not published as text.
Questions and answers
What is the only unique charm?
Gheed's Fortune, at level 70. Only one copy works.
Can a shop sell charms?
No. The items page says charms only drop. Shops and gambling do not produce them.

Source scope: the official site, FAQ, wiki and changelog share one publisher. Build IDs, installer sizes and detailed mechanics are single-source official claims unless a separate independent reference is identified. Several official links do not count as independent confirmation.
